Application Process

- Fill out the application. Once you electronically sign your application and submit it, you will receive an email asking you to confirm your signature. Please be sure to follow the directions to confirm your signature.
- All participants must download and print the Medical History/Physicians Statement. Those receiving Physical Therapy should also download and print the Physicians Prescription Form. Please bring the form(s) with you to your screening appointment.
